as a participant in any and all TWC “Activities,” including, but not limited to, wrestling camps, clinics, practices, conditioning, drilling, instruction, work-outs, duals, tournaments, swimming, other free play, travel to/from said Activities, overnight stays at hotels, resort activities and in return for the opportunity to participate in said Activities:
It is agreed that any and all risks attendant to watching, participating, overnight stays, traveling to and from said Activities, including, but not limited to, bodily injury, illnesses, infections/diseases (including skin-type) and COVID-19 infection/disease, are assumed by the participant and his/her parents and/or legal guardians and that this assumption is acknowledged, approved, and agreed to by said participant and his/her parents and/or legal guardians as indicated by the signatures hereto.
The participant and/or his/her parents and/or legal guardian(s) will be financially responsible for and have insurance that will cover injuries/accidents/diseases/illnesses occurring during these Activities.
I hereby certify that the above-named participant is physically able to participate in any and all TWC Activities and that I know of no physical impairments, illnesses, infections or diseases which would in any manner limit his/her participation in such Activities.
I hereby grant permission for emergency medical care by physicians, dentists, or other licensed health care providers to administer outpatient medical, surgical, or dental services as appropriate or necessary antigens or other injections, to perform such emergency procedures as necessary or to refer to other duly licensed medical personnel when necessary, when participating in TWC Activities. Parents and/or next of kin will be promptly notified by TWC in the event emergency medical care is necessary.
In consideration for honoring the participant’s request to participate in TWC Activities, I, for myself, my executors, administrators, and assigns, do hereby release and forever discharge Titan Wrestling Club, Inc., and its Board of Directors, Officers, its respective entities, administrators, employees, agents, coaches, and instructors from any and all claims that I might have myself or could bring on the participant’s behalf with regard to damages, demands, or any actions whatsoever, including those based on negligence or failure to supervise, in any manner arising out of the participant’s participation in these Activities. I also hereby agree to save, hold harmless, and indemnify Titan Wrestling Club, Inc., its Board of Directors, Officers, and/or its respective entities, administrators, employees, agents, coaches, and instructors against any and all claims, including claims of negligence or failure to supervise, that myself or the participant might bring against them as a result of his or her participation in these Activities. I recognize that this Release means that I am giving up, among other things, rights to sue Titan Wrestling Club, Inc., and its Board of Directors, Officers, its respective entities, administrators, employees, agents, coaches, and instructors for injuries, damages, or losses that myself or my child may incur.
